{"id":10681,"date":"2020-12-05T01:06:01","date_gmt":"2020-12-05T01:06:01","guid":{"rendered":"https:\/\/www.seasidemedicalpractice.com\/dev\/\/dev\/?p=10681"},"modified":"2020-12-05T01:06:01","modified_gmt":"2020-12-05T01:06:01","slug":"nad-infusion","status":"publish","type":"post","link":"https:\/\/www.seasidemedicalpractice.com\/dev\/nad-infusion\/","title":{"rendered":"NAD+ Infusion"},"content":{"rendered":"<p class=\"p1\"><b>Nicotinamide Adenine Dinucleotide aka NAD<\/b><span class=\"s1\"><b><sup>+<\/sup><\/b><\/span><\/p>\n<p class=\"p3\">At Seaside, we pride ourselves for discovering and implementing breakthrough therapeutics in the clinical setting, and we postulate that NAD<span class=\"s2\"><sup>+<\/sup><\/span> supplementation is the emerging wonder in the world of Anti-Aging and Cognitive Prowess.<span class=\"Apple-converted-space\">\u00a0 <\/span>A great body of evidence coupled with advancements in pharmaceuticals has facilitated our ability and strengthened our confidence in offering intravenous infusion of NAD<span class=\"s2\"><sup>+<\/sup><\/span> to our patient population.<span class=\"Apple-converted-space\">\u00a0 <\/span>We now have a viable option to restore energy, vitality and mental clarity in the face of physiologic and environmental stressors.<\/p>\n<p class=\"p1\"><b>What is NAD<\/b><span class=\"s1\"><b><sup>+<\/sup><\/b><\/span><b>? <\/b><\/p>\n<p class=\"p3\">NAD<span class=\"s2\"><sup>+ <\/sup><\/span>is synthesized in the cells with NAD<span class=\"s2\"><sup>+<\/sup><\/span> precursors such as tryptophan, and different forms of Vitamin B3.<span class=\"Apple-converted-space\">\u00a0 <\/span>Tryptophan builds NAD<span class=\"s2\"><sup>+<\/sup><\/span> de novo through the kynurenine pathway.<span class=\"Apple-converted-space\">\u00a0 <\/span>Tryptophan is also the starting substrate for serotonin and melatonin production (hence why that Thanksgiving turkey made you sleepy).<\/p>\n<p class=\"p3\">Vitamin B3 consists of nicotinic acid found in plants and algae, nicotinamide, a by-product of pyridine nucleotide, and nicotinamide riboside found in milk.<span class=\"Apple-converted-space\">\u00a0 <\/span><b>Fig 1<\/b><\/p>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"alignnone size-medium wp-image-10683\" src=\"https:\/\/www.seasidemedicalpractice.com\/dev\/\/dev\/wp-content\/uploads\/2020\/12\/NAD-synthesis--300x271.jpg\" alt=\"NAD infusion\" width=\"300\" height=\"271\" \/><\/p>\n<p class=\"p3\">These vitamins build NAD<span class=\"s2\"><sup>+<\/sup><\/span> with help of several other factors, including the gut microbiome.<\/p>\n<p class=\"p1\"><b>What are the biological roles of NAD<\/b><span class=\"s1\"><b><sup>+<\/sup><\/b><\/span><b>?<\/b><\/p>\n<p class=\"p3\">Many processes in the body depend on NAD<span class=\"s2\"><sup>+<\/sup><\/span> including cholesterol synthesis, glutathione generation, DNA repair and gene expression, adaptive stress responses, maintenance of calcium homeostasis, ATP generation and most importantly control of lifespan through the sirtuin pathway.<\/p>\n<p class=\"p3\"><b>ENERGY<\/b> &#8211; One way NAD<span class=\"s2\"><sup>+ <\/sup><\/span> controls cellular energy is by serving as a reservoir for hydrogen ions involved in REDOX reactions in the mitochondria.<span class=\"Apple-converted-space\">\u00a0 <\/span>The REDOX reaction produces ATP, the \u201cenergy currency\u201d of the body.<span class=\"Apple-converted-space\">\u00a0 <\/span>Without adequate stores of NAD<span class=\"s2\"><sup>+<\/sup><\/span>, this reaction slows down and ATP production declines. Perturbations in mitochondrial function leads to genomic instability which ultimately leads to cell demise.<\/p>\n<p class=\"p3\"><b>AGING<\/b> &#8211; In addition to being the regulator of metabolic processes in the body, NAD<span class=\"s2\"><sup>+<\/sup><\/span> also aides sirtuins, a class of enzymes that regulate our genes and protect the cells from dying prematurely.<span class=\"Apple-converted-space\">\u00a0 <\/span>Sirtuins play a pivotal role in longevity and accelerated aging, and they need an optimal amount of NAD<span class=\"s2\"><sup>+<\/sup><\/span> to function.<\/p>\n<p class=\"p3\">With over 500 processes in the body dependent of adequate concentrations of NAD<span class=\"s2\"><sup>+<\/sup><\/span>, it is no wonder it has become a salient area of research for targeted therapies towards degenerative diseases such as Alzheimer\u2019s and chronic inflammatory conditions.<\/p>\n<p class=\"p1\"><b>What reduces NAD<\/b><span class=\"s1\"><b><sup>+<\/sup><\/b><\/span><b> production? <\/b><\/p>\n<p class=\"p3\">When the production of NAD<span class=\"s2\"><sup>+ <\/sup><\/span>declines or the consumption is too high, there is an imbalance of supply and demand.<span class=\"Apple-converted-space\">\u00a0 <\/span>The demand may be acute such as excessive DNA damage from free radicals or UV attack, or from chronic inflammatory states.<span class=\"Apple-converted-space\">\u00a0 <\/span>Accumulation of oxidative stress in the aged population also depletes NAD<span class=\"s2\"><sup>+<\/sup><\/span> stores.<span class=\"Apple-converted-space\">\u00a0 <\/span>High fat diet, deficiency in vitamin B3 or B6 or tryptophan decrease production of NAD<span class=\"s2\"><sup>+<\/sup><\/span>.<\/p>\n<p class=\"p1\"><b>What increases NAD<\/b><span class=\"s1\"><b><sup>+<\/sup><\/b><\/span><b> production? <\/b><\/p>\n<p class=\"p3\">Caloric restriction, exercise, glucose deprivation, resveratrol, adequate intake of vitamin B3, vitamin B6 and tryptophan, as well as NAD<span class=\"s2\"><sup>+<\/sup><\/span> precursors such as NMN or NR.<\/p>\n<p class=\"p3\">In a seminal paper on NAD<span class=\"s2\"><sup>+<\/sup><\/span> released in January 2019, the authors postulate that NAD<span class=\"s2\"><sup>+ <\/sup><\/span>supplementation may lead to beneficial effects on brain, cardiac, and renal function under various stressors, and that <i>intravenous administration of NAD<\/i><span class=\"s2\"><i><sup>+<\/sup><\/i><\/span><i> is the only effective means of clinically increasing system NAD<\/i><span class=\"s2\"><i><sup>+<\/sup><\/i><\/span><i> levels.<\/i><span class=\"s3\"><i><sup>1<\/sup><\/i><\/span><\/p>\n<p class=\"p1\"><b>What is the process of NAD infusion? <\/b><\/p>\n<p class=\"p3\">NAD comes in an injectable form or diluted in solution for intravenous delivery.<span class=\"Apple-converted-space\">\u00a0 <\/span>The injection may be administered at home intramuscularly with a 25 gauge needle.<span class=\"Apple-converted-space\">\u00a0 <\/span>The intravenous solution contains 400mg of pure NAD<span class=\"s3\"><sup>+<\/sup><\/span> which is then infused over 2- 2.5 hours.<span class=\"Apple-converted-space\">\u00a0 <\/span>The effects are immediate, and may be repeated every week or every month thereafter.<span class=\"Apple-converted-space\">\u00a0 <\/span>Treatment intervals are yet to be elucidated, and likely will depend on each individual\u2019s response.<span class=\"Apple-converted-space\">\u00a0 <\/span>Adverse effects during the infusion are nausea, shortness of breath, chest tightness and rapid heart rate.<span class=\"Apple-converted-space\">\u00a0 <\/span>These effects subside while receiving the infusion or shortly thereafter.<\/p>\n<p class=\"p3\">Candidates most suitable for this procedure are essentially anyone interested in boosting cellular energy metabolism in their body.<span class=\"Apple-converted-space\">\u00a0 <\/span>Those with chronic illness, chronic inflammatory condition, accelerated aging, elderly, athletes, fatigue, addiction may benefit the most from even just one session.<\/p>\n<p class=\"p3\">My experience after a single session of intravenous NAD<span class=\"s2\"><sup>+<\/sup><\/span> was profound.<span class=\"Apple-converted-space\">\u00a0 <\/span>It was like pressing the boost button on my brain.<span class=\"Apple-converted-space\">\u00a0 <\/span>I had an immediate sense of wellbeing, energy, mental clarity, vision enhancement, and a renewed zest for life!
