Your First Visit
Your first visit to Seaside Medical Practice will begin with introductions to our staff, and will include a thorough evaluation of your health and/or medical condition. We'll address any immediate medical needs you may have and will work with you to establish a follow up plan of care as necessary.
During your first visit we'll collect some information from you (forms available below), collect your initial payment or co-pay, and will schedule any necessary follow up visits if required.
Following your visit, your physician will document your session and will communicate with your referring physician, and or any specialists as necessary.
Forms
All new patients to Seaside Medical Practice must complete a New Patient Forms Packet which provides us with the information necessary to understand your condition and how we can best help you. Information obtained in our New Patient Forms Packet also allows us to properly bill your insurance for services rendered at the clinic. At Seaside Medical Practice, our service to you is our top priority. In order to ensure that we can provide the highest quality of care possible, it is important for us to collect payments for service in a timely manner.
