Refill Requests

If you have a refill request, please phone your request to our main office at (310) 393-5000. Please provide your full name, the name of your medication, the pharmacy at which you would like to pick up your refill, and your contact phone number for follow up questions with regard to your request. Alternatively, you may access our office through HIPPA secure portal, MyMedicalLocker.com. You will be given an access code by our office to facilitate your account creation. Please note, our office does not respond to general email inquiries as of January 1st, 2018 due to federal electronic communications compliance mandates.

Records Requests

If you would like to receive a copy of your medical records, please phone our main office at (310) 393-5000, or alternatively you may send your request through the HIPPA secure portal MyMedicalLocker.com. You will be given an access code by our office to facilitate your account creation.
Please provide your full name, date of birth and contact information. All records will be sent to you either by mail, fax or pick up. We are no longer able to transmit medical records in electronic format to any non-HIPPA secure email address.

Appointment Requests

If you would like to schedule an appointment at Seaside Medical Practice, please phone our main office at (310) 393-5000 or access our office through MyMedicalLocker.com. Please provide your full name, the reason for your appointment, and your requested day and time for your appointment.  IMPORTANT: NEVER RELY ON ELECTRONIC FORMS OF COMMUNICATION IN AN EMERGENCY.  IF YOU ARE HAVING A MEDICAL EMERGENCY, PHONE 911 IMMEDIATELY FOR HELP.

Billing/Insurance Questions

If you are an existing patient and have questions regarding a recent statement or bill for services rendered by Seaside Medical Practice, please contact our billing department at (310) 393-5000 ext. 8.

If you are not a patient of record, and have questions regarding insurances accepted, pre-authorizations, or services rendered, please phone our main office at (310) 393-5000, or feel free to contact us by email at insurance@seasidemedicalpractice.com.
