Refill Requests
If you have a refill request, please phone your request to our main office at (310) 393-5000 or email us at refills@seasidemedicalpractice.com. Please provide your full name, the name of your medication, the pharmacy at which you would like to pick up your refill, and your contact phone number and email address for follow up questions with regard to your request.
Records Requests
If you would like to receive a copy of your medical records, please email us at records@seasidemedicalpractice.com. Please provide your full name, date of birth and contact information. All records will be sent to you in electronic format to the email address provided upon registration in our office. If you would like to have your records emailed to a different address, faxed or sent via regular mail, please phone our main office at (310) 393-5000.
Appointment Requests
If you would like to schedule an appointment at Seaside Medical Practice, please phone our main office at (310) 393-5000 or email us at appointments@seasidemedicalpractice.com. Please provide your full name, the reason for your appointment, and your requested day and time for your appointment. IMPORTANT: NEVER RELY ON EMAIL COMMUNICATION IN AN EMERGENCY. IF YOU ARE HAVING A MEDICAL EMERGENCY, PHONE 911 IMMEDIATELY FOR HELP.
Billing / Insurance Questions
If you are an existing patient and have questions regarding a recent statement or bill for services rendered by Seaside Medical Practice, please contact our billing department at (877) 780-9202.
If you are not a patient of record, and have questions regarding insurances accepted, pre-authorizations, or services rendered, please phone our main office at (310) 393-5000, or feel free to contact us by email at insurance@seasidemedicalpractice.com.
